For the first time, at one location, a fully integrated model of services combines individualized counseling, support, medication management, education, training, and case management services for adults struggling with mental health and substance abuse. The multi-faceted approach is designed to address each person's behavioral health needs while empowering consumers and their families with the skills needed to strengthen family cohesiveness.The program's purpose is to provide comprehensive outpatient mental health services to adults ages 18 and over, adult couples, and families in order to alleviate symptoms of psychological distress, enhance coping skills, promote psychiatric stability, and improve functioning in all aspects of life.

Case Management
All consumers are assigned a Case Manager by the Team Leader. Our Case Management services is by far the best, our team is comprised of specially qualified health professional that assist the consumer on focusing on themselves, their strengths, and their weaknesses. Case Managers assist individuals with becoming stable in their community helping to eliminate multiple hospitalizations through medication management and education. In achieving member wellness and autonomy through advocacy, communication, education and service facilitation. Case Managers goal to prevent inpatient hospitalization and decrease the frequency and severity of acute episodes.
Individual Therapy
Our individual therapy is a comprehensive therapy program that provides therapy to the severe and persistently mentally ill. The program is open-ended. It operates Monday through Friday and transportation can be arranged.
The Treatment Plan Coordinator facilitates development, and subsequently reviews client treatment plans. The goal of the Outpatient Medical Department is to improve the quality of life by ameliorating severe and painful symptoms and to preserve life when the effect of mental disorder threatens it.
Assertive Community Treatment (ACT)
The goal of our ACT team is to provide an intensive management - care program that will increase independent functioning for people with histories of psychiatric hospitalization and treatment resistance, including those who also suffer from substance abuse. AFFCS Behavioral & Wellness Services staff are available 7 days a week and on call 24 hours a day.
Our intensive Case Management provides intensive services to individuals diagnosed with mental illness of such a magnitude that it significantly impairs their day-to-day personal, social, vocational, and educational functioning. An integral part of our treatment is to assist consumers in learning basic living skills, taught to d their hygiene, meal preparation, food services, and clean-up duties, if appropriate or if needed.
